Insurance and payment options

Insurance:

We accept most insurance plans. We also accept payment from most PPO and indemnity plans along with in-office payment plans for your convenience. Please call our office to make an appointment and to ask any questions you may have.

Payment Options:                             

We accept all major credit cards, ATM cards, cash and personal checks.     
For our patients' convenience we have arranged a payment plan through a third party that helps our patients receive their treatment in a timely manner.

In-Office Insurance Plan - An Invitation for Membership

 Dentistry by Design is pleased to invite you to become a member of our personalized dental benefits plan.  For the cost of $125.00 per year, you will receive the following:

Ø      Routine office visits and consultations to keep you in excellent dental health.   --$25.00 per visit.

Ø      Dental cleanings and exams every six months, including x-rays for adults and fluoride treatments for children.  --$25.00 per visit.

Ø      Emergency exam and diagnostic x-ray.  --$25.00 per visit.

Ø      Reduced fees (20% or more) on ALL needed dental work.

Ø      Very gentle dentistry for adults and children with an emphasis on high quality care.

Notes

We urge you to keep any HMO or managed care medical insurance you now have.  You may need it for medical/ hospital expenses, pharmacy expenses, etc.

Your membership is only valid at our office. You may want to keep any other dental insurance you have in case you move out of the surrounding area or if you require a dental specialist. 

Our office does accept most major dental insurances; however our customized membership plan will provide you with the quality dentistry that you deserve. 

This invitation is made available to patients that do not carry dental insurance or do not wish to utilize it, but are interested in a discounted membership program.  (This membership may not be used as a secondary insurance.)
